Because every patient has different needs, we’ve designed three levels of membership to give you the right balance of access, support, and personalization. Each tier includes extended visits, functional medicine testing, and ongoing communication with your Nurse Practitioner.
Wellness Level: Tier 1 is perfect for patients who want foundational functional medicine support and regular check-ins. 2 visits /year included.
Prevention Level: Tier 2 offers added flexibility with 6 visits/year, messaging, and deeper access to personalized care.
Concierge Level: Tier 3 is designed for those who want the highest degree of partnership — with priority scheduling, enhanced coordination, and elevated support of 10 visits/year.
